HRT cost calculator: the first-year number

The cheapest all-in first-year path on the women's wing is Alloy at about $530, Transparency Grade A. Every SKU priced before intake, the category's transparency benchmark. On the men's wing the cheapest all-in path is MangoRx at about $1,188. Both figures are a year, and that is the point of this tool: the consult fee, the required 12-month plan, the three-month billing block and the membership that buys no medication all disappear from a monthly sticker and all reappear in the annual total.

The calculator will not rank a per-visit fee against a monthly subscription, and it will not add a medication estimate to a provider that bills the drug elsewhere. Those numbers are not published, and a guess would make every group look comparable while making every total wrong. Every assumption it does make is printed below, on screen, before you read a single figure.

Every assumption behind the numbers below

  • Twelve months. A monthly price becomes a year-one total by multiplying by 12. No provider on this roster prices a partial year.
  • Per-visit providers use 3 visits. You set that number above, and 3 providers in the current view are priced this way. Each visit is charged at the published follow-up rate. The first visit costs more at every per-visit provider on this roster, and that figure is on each one's review page. Our total therefore runs low for a first-year patient by the difference between the two rates.
  • Documented paths override the arithmetic.Where a provider carries a researched year-one figure, that figure is used and its assumption note is printed on the row. Those numbers come from a specific route through the provider's own price list, usually its cheapest systemic-estrogen option.
  • No invented medication cost. Providers in the care-only group bill the drug through your pharmacy and publish no price for it. We add nothing, and we label the group instead.
  • No lab estimate. Where a provider publishes no self-pay lab price, we print the absence rather than a placeholder.
  • Nothing is stored. Your selections stay in this browser tab and are never sent anywhere.

Cash pay is not automatically the expensive tier. Alloy at $530 for year one beats plenty of copay stacks once the pharmacy price of brand HRT is counted. Of the 13 platforms in this wing, 10 are cash-pay only.

All-in, month to month

The published price buys the care and the medication, with no annual commitment. These totals are directly comparable with each other and they are the only group where a low number means what it looks like it means.

All-in, but only on an annual or 12-month plan

The medication is included at the advertised rate only if you commit to a year. The year-one total is real. The monthly figure beside it is an anchor, and leaving early usually costs more per month than the headline.

Oral estradiol at $79/mo on the required 12-month plan = $948, medication included. Patch route: $134/mo × 12 = $1,608. Shorter plans cost more; exact prices gated behind intake.

Care only, medication billed somewhere else

These totals buy the clinician and nothing else. Your prescription goes to your own pharmacy or to a third party, and none of these providers publishes a medication price we could verify. Add your pharmacy's number to every figure in this group before comparing it with the groups above.

What a monthly price leaves out

Four pricing mechanics live in the gap between the sticker and the year, and all four are documented on this roster. A one-time consult fee is charged before the first monthly charge. An advertised rate can require a 12-month commitment, so the low number and the escape hatch cannot both be true. Prescriptions can bill as three-month supplies, which triples the amount that leaves your account per charge without changing the monthly figure at all. A membership can be a real recurring charge that buys no medication, so the drug is a second bill you have not seen yet.

A year absorbs all four. The static version of this table, with each figure's sourcing written out and the per-provider verification dates attached, is how much does HRT cost. If you want the lowest published number rather than the lowest total, that is the cheapest HRT table for the women's wing and cheapest TRT for the men's. Insurance specifics sit on the HRT insurance page.

This is general information, not medical advice. HRT is a prescription treatment and the right route, dose and provider are decisions for you and a licensed clinician. Every figure here is an estimate built from published prices and the stated assumptions, and your actual bill depends on your prescription, your pharmacy and your plan. Reviewed by Iacob Pastina.
