Bleeding or Spotting on HRT: What's Normal and What Needs a Workup

Six weeks into HRT, you are spotting, and the leaflet in the box tells you to report "unusual vaginal bleeding" to your clinician right away. The leaflet never defines unusual. That gap is where most of the 2 a.m. searching happens, so here is the split up front.
Irregular bleeding or spotting during the first months of a continuous combined regimen (estrogen plus a progestogen every day) is expected and documented on the products' own FDA labels. It fades: in the Activella trial, about 86 percent of women were bleed-free after 12 months of treatment. A scheduled monthly bleed on a cyclic regimen is the regimen working as designed. The bleeding that warrants a prompt workup is different in pattern: bleeding that persists past the settling window, bleeding that returns after you have been bleed-free on HRT, and any bleeding after menopause if you are on no hormones at all. The workup exists to rule out endometrial cancer, and in roughly 9 of 10 women it finds none.

Is spotting normal when you start HRT?
On a continuous combined regimen, yes, for a while. This is one of the few side-effect questions where the FDA labels publish an actual curve with numbers on it. The Prempro patient labeling lists "irregular vaginal bleeding or spotting" among the common side effects, and the Activella label goes further and describes the curve: "During the initial months of therapy, irregular bleeding or spotting occurred... However, bleeding tended to decrease over time, and after 12 months of treatment with Activella 1 mg/0.5 mg, about 86 percent of women were amenorrheic," meaning no bleeding or spotting at all.

Read the Bijuva rows honestly and the scale of "normal" becomes concrete: on the standard 1 mg/100 mg dose, more than 4 women in 10 had at least some bleeding or spotting during their first year. Even on placebo, about 1 in 5 postmenopausal women reported some bleeding or spotting across the year. Spotting early on tells you very little by itself. The pattern over months is the signal, and it is the reason the red-flag rules below are written as changes in pattern.
One honest gap: these curves exist because FDA-approved products are required to publish them. Compounded hormone creams and pellets are not FDA approved and carry no label, so there is no equivalent bleed chart for them. Why does HRT cause bleeding in the first place?
Estrogen makes the uterine lining grow. That single fact drives the whole design of HRT for a woman with a uterus: unopposed estrogen raises the risk of endometrial hyperplasia and endometrial cancer, and the Prempro label states plainly that "adding a progestin to estrogen therapy has been shown to reduce the risk of endometrial hyperplasia, which may be a precursor to endometrial cancer." The progestogen in your regimen is there to keep the lining thin and stable. Spotting in the early months is that push and pull playing out in tissue that spent years without either hormone. Dose is part of it too: in Bijuva's trial, vaginal bleeding was reported as an adverse reaction by 3.4 percent of women on the 1 mg estradiol dose against 2.4 percent on the 0.5 mg dose, and no women on placebo. Consistency is the other part: the Bijuva label's own counseling section tells women who miss the evening dose to take it with food as soon as they can, unless the next dose is within two hours: a regimen taken erratically gives the lining an erratic signal.
Why do I get a monthly bleed on cyclic HRT?
Because the regimen schedules one. On a cyclic (sequential) regimen, estrogen runs every day and the progestogen runs part of the cycle: Prometrium's label doses 200 mg at bedtime for 12 days per 28-day cycle, and Premphase packages estrogen for days 1 through 28 with the progestin added on days 15 through 28. During the progesterone days the lining converts toward its secretory form (in the label's study of estrogen-primed postmenopausal women, 10 days at 400 mg induced complete secretory change in 45 percent, against none on placebo). When the progesterone stops, that lining sheds. The result is a withdrawal bleed on a schedule.
The label carries hard numbers for how reliably progesterone withdrawal produces a bleed: in a randomized study of women with absent periods, 80 percent bled within 7 days of the last Prometrium dose, against 10 percent on placebo, and a larger follow-up study put it at 73.8 to 76.8 percent. Those studies were in premenopausal women, so treat the exact percentages as mechanism evidence, and the mechanism is the one your cyclic regimen uses on purpose. A predictable bleed arriving near the end of, or just after, your progesterone days is the regimen working. A bleed that turns up at random points in the cycle, or gets much heavier than your established pattern, belongs in the next section.

When is bleeding on HRT a red flag?
Here is the rule, stated without softening and without alarm. Three patterns warrant a prompt evaluation:
- Bleeding that persists past the settling window. The label curves above show bleeding on continuous regimens decreasing through the first year, with most women bleed-free by month 6 to 12. Bleeding that is still frequent as you approach the one-year mark, or is getting heavier instead of lighter, has left the expected pattern.
- New bleeding after a bleed-free stretch on HRT. If you were amenorrheic on continuous HRT and bleeding returns, that is a change in pattern, and it is exactly what the estrogen labels mean by "recurring abnormal genital bleeding."
- Any postmenopausal bleeding when you are not on HRT. Off hormones, there is no regimen to blame the bleeding on. The Prempro patient leaflet puts it in one sentence: "Vaginal bleeding after menopause may be a warning sign of cancer of the uterus (womb). Your healthcare provider should check any unusual vaginal bleeding to find out the cause."
The reason for the rule is the endometrium. The full prescribing information instructs that "adequate diagnostic measures, including directed or random endometrial sampling when indicated, should be undertaken to rule out malignancy in postmenopausal women with undiagnosed persistent or recurring abnormal genital bleeding," and undiagnosed abnormal genital bleeding is a listed contraindication to starting these products at all. The label's timing instruction is equally plain: report abnormal bleeding to your clinician as soon as possible. This matters because bleeding is how endometrial cancer announces itself: it is the presenting sign in more than 90 percent of postmenopausal women who have it, which makes bleeding the one early-warning system this cancer offers.
Now the calibration, because the base rates cut the other direction. A 2018 meta-analysis in JAMA Internal Medicine covering 129 studies and over 34,000 women with postmenopausal bleeding put the pooled risk of endometrial cancer at 9 percent (95% CI, 8 to 11 percent), and in the analyses split by hormone therapy use the estimates ranged from 7 to 12 percent. Roughly 9 in 10 women who get this workup walk out without a cancer diagnosis. That number is the reason to book the appointment calmly, and it is never a reason to skip it.
The rule in one line
Spotting in the first months of continuous HRT: expected, track it, raise it at your review. Bleeding that persists past the settling window, returns after a bleed-free stretch, or occurs off HRT: book an evaluation now. Most of these workups find no cancer; the evaluation is how you find out.

What will the doctor actually do about bleeding on HRT?
Less than most women fear. For an initial episode of postmenopausal bleeding, ACOG's committee opinion endorses transvaginal ultrasound as a reasonable first step: a probe ultrasound that measures the thickness of the endometrial lining in millimeters. A measurement of 4 mm or less carries a greater than 99 percent negative predictive value for endometrial cancer, which is why a thin-lining result typically ends the cancer question for an initial episode. The alternative first step is an office endometrial biopsy, a sampling of the lining.
Two follow-on rules from the same document shape what happens next. If bleeding persists or returns after a negative biopsy, ACOG says further testing such as hysteroscopy with dilation and curettage is warranted, so a normal first result does not close the file on continued bleeding. And the door swings the other way too: a thickened lining found incidentally on a scan done for some other reason, in a woman with no bleeding, does not routinely need a workup, though ACOG leaves room for an individualized assessment based on her risk factors. The trigger is the bleeding, and the follow-through is the pattern.
Do bloating and breast tenderness on HRT go away?
These two travel with the early months the same way spotting does, and the placebo-controlled numbers put honest boundaries on them. In Prometrium's three-year trial of cyclic progesterone with conjugated estrogens, breast tenderness was reported by 27 percent of women against 6 percent on placebo, and abdominal bloating by 12 percent against 5 percent. Headache, which women often blame on the hormones, barely separated from placebo: 31 percent against 27. So the bloating and breast signal is real and drug-related, and it still leaves the large majority of women unaffected.
Dose moves both. In Bijuva's trial, breast tenderness ran 10.4 percent on the 1 mg estradiol dose, 4.0 percent on the 0.5 mg dose, and 0.7 percent on placebo. The Prempro label's HOPE study shows the same staircase: breast pain fell from 26 to 20 to 13 percent as the dose stepped down, against 8 percent on placebo. A clean dose-response is useful news, because it means tenderness that will not quit is a lever your prescriber can actually pull. The progestogen contributes too: on the Activella label, breast pain was reported by 24 percent on the combined pill against 10 percent on the same estradiol dose alone. The checkpoint is three months. The pivotal symptom trials behind these labels measured their vasomotor endpoints at 12 weeks, which makes the 3-month mark the natural review point: it is when the trial evidence itself expects the picture to be readable. If bloating or breast tenderness has not eased by your 3-month review, bring the dose-response numbers above to the conversation. One adjacent trap: bloating is routinely experienced and reported as weight gain, while in the randomized PEPI trial women on hormone therapy averaged 1.0 kg less weight gain than placebo over 3 years. How do you stop spotting on HRT without making it worse?
The first rule is the one that protects you: do not drop the progesterone on your own to see if the spotting stops. Unopposed estrogen in a woman with a uterus raises endometrial hyperplasia risk, which is the exact outcome the progesterone is in your regimen to prevent. Spotting is a nuisance; a thickened, unopposed lining is a medical problem. Every change worth making here runs through the prescriber.
- Take the regimen consistently. Same time, every day, per the label's own missed-dose instructions. An erratic hormone signal is the one spotting contributor entirely in your hands.
- Give a continuous regimen its documented window. The label curves run 6 to 12 months. Quitting at week eight because of spotting means quitting inside the period the manufacturer's own data marks as the adjustment phase.
- Track dates and heaviness. The red-flag rule is about pattern, and a dated log is what turns "I think it's been worse lately" into something a clinician can act on at the 3-month review.
- If it persists past the window, the evaluation rule applies. Dose or regimen changes are the prescriber's call, and persistent bleeding earns the workup described above before anyone shrugs at it.
